Nissan Fundoplication

The most common surgery for reflux is the Nissan Fundoplication.  Basically, the surgeon frees up the upper portion of the stomach, then uses it to wrap around the lower portion of the esophagus.

This wrap creates a high pressure zone at the end of the esophagus, making it hard to spill acid into the esophagus. In this sense it helps with the mechanical aspect of reflux, but does not change the chemical aspect. The fluid remains acidic, but the fluid does not get into the esophagus. Manometry helps to predict how successful this surgery is, and studies indicate over 90% of patients significantly improve their reflux. Although symptoms improve, many patients will still use medications to control the acid content of the stomach. This is more common is cases of gastritis and gastric ulcers.
